- Chronic simple laryngitis: intermittent voice hoarseness that is worst right after waking, gets better as the person becomes more physically active, then returns again by the following morning. Across extended periods the voice grows fatigued and speech becomes impossible to sustain for a long stretch. Speaking produces throat soreness and discomfort. The patient frequently clears the throat out of habit, yet no visible sputum is produced. Hoarseness stands out most clearly when a high-pitched tone is attempted. The laryngeal mucosa shows widespread congestion. The vocal cords lose their normal pearly-white hue, shifting to pink or advancing to a darker red. The vertical capillaries running alongside the vocal cord margins become enlarged, and the margin itself thickens. Either the glottis fails to close fully, or the vibration on both sides is uneven. The outer surface of the vocal cords is commonly coated in a sticky discharge that forms silk-like threads bridging the space between the cords.
- Chronic hypertrophic laryngitis: recurring hoarseness most pronounced immediately after rising, easing as activity picks up over the day, only to reappear again by the next morning. Given enough time, the voice becomes worn out and cannot be sustained for a prolonged period.
- Speaking causes throat soreness and pain. There is frequently a habitual urge to clear the throat despite no visible sputum being brought up. A high-pitched sound attempt makes the hoarseness stand out more clearly. Patients report a long-standing history of hoarseness progressing toward loss of voice. The hoarseness is quite pronounced while any cough tends to be mild. From time to time acute exacerbations occur, worsening the hoarseness further and degrading voice quality. The laryngeal mucosa swells on both sides, though not evenly — the swelling is most pronounced in the arytenoid region, where it rises prominently. Producing a sound creates one or more folds in the arytenoid area. The vocal cord thickens and takes on a rough, stick-like appearance rather than a smooth one, frequently developing a nodular or polyp-like shape coated with a sticky discharge. Should the condition extend to the region beneath the glottis, the vocal cords are kept from meeting and triangular gaps develop. Should the condition extend to the region beneath the glottis, the vocal cords are kept from meeting and triangular gaps develop. The ventricular folds enlarge and often push against the vocal folds during phonation. The aryepiglottic fold can also thicken, though enlargement of the epiglottis itself is very uncommon.
